Formal Requests for Information
If you’re unable to find the information you need through our health records department, through other information sources or through an informal request, you are within your rights to submit a formal Freedom of Information Request and providing a written statement of the information you are requesting.
Call out form: FIPPA requests typically require a fee that is used to compensate for staff time and materials. If you’re request will cost over $30.00, we will reach out to let you know. The more detail you provide about what specific information you are seeking the more efficiently our team can do so without incurring additional cost to the requester.
