As per our health centre’s policy, medical records older than 15 years are no longer available.
If you are requesting a copy of a health record from the health centre for a patient who has passed away, you will be required to provide proof of your legal authority to do so along with an Request for Release Form (PDF) to our Health Records department.
You must submit one of the following:
- A copy of the deceased person’s Will naming you as the Executor, or
- If there is no Will, a notarized letter from a lawyer confirming your legal signing authority.
