For more than 50 years, communities in the West Parry Sound area have operated “nursing stations” to meet the health care needs of our rural population. However, gone are the days when a single nurse working alone in a station could keep pace with today’s preventative practices, chronic disease management, and health education demands. Health care delivery has changed significantly in the past few decades. What once worked as a basic, walk-in-style “nursing station” no longer fully meets the complex needs of our communities. People today are living longer, facing multiple chronic conditions, and benefiting from evidence-based care. By transitioning to NPLCs, we are ensuring that our rostered patients have access to, and a relationship with, a regular team of healthcare providers. Our patients have a “medical home.” The quality of care, safety, and long-term benefits for our community are significantly enhanced under this team-based, primary care, NPLC model.
