The WPSHC Diabetes Education Program is an outpatient program staffed by Certified Diabetes Educators, including a Register Nurse and a Registered Dietitian.

The Diabetes Education Program is available for individuals that are newly diagnosed with type 2 diabetes, prediabetes and gestational diabetes as well as for follow up of type 1 diabetes, type 2 diabetes, diabetes in pregnancy, individuals on basal bolus insulin regimes or insulin pumps and for those classified as ‘at risk’ for diabetes.
- Patients are able to self-refer to this program by calling (705) 746-4540 ext. 1302.
- This program is a free service to those with an OHIP card.
- Participants are encouraged to bring a support person to their appointments.
Frequently Asked Questions
Clients require a diagnosis of diabetes or prediabetes to participate in this service. Clients classified as high risk by their health care provider may also qualify for this service.
You can either be referred by your primary health care provider, or you can self-refer by calling 705-746-4540 ext. 1302
Individuals will receive one-on-one counselling on the self-management of diabetes.
You may be provided with a blood glucose meter if you require one, you will be instructed on use and how often to test your blood glucose as well as target blood glucose values.
- Blood glucose meter (if you have one).
- A list of current medication list.
- A 3-day diet history.
How do I get to?
- Directly across from the entrance you will find the Unit Reception Desk, speak with the clerical staff.
- Head down to the 1st floor via the stairs or elevator directly across from the entrance.
- Once on the first floor find Diabetes Care reception on the right-hand side of the stairs or across from and to the left of the elevators.
